--- Any drug allergies? What happens when you take that drug?

--- Have you ever been hospitalized?
--- Have you had any surgeries?
--- When was your last examination?
Accidents or injuries (WE might SKIP THIS?)
--- Any accidents or injuries?
Childhood illnesses (I THINK WE SKIP THESE???)

regarding medications- do you take your medication as it is prescribed? katie
in regards to Hx of respiratory/cardiac disease... might it be a good idea to ensure that our patients understand what we mean by that (i was watching a soccer movie the other day where the guy didn't understand what the nurse meant by respiratory disorders and thus failed to mention his asthma). i mention this in particular because furosemide is used to CHF which is characterized by SOB... not to jump the gun and try to diagnose.. katie

as far as childhoos illnesses....do you have a Hx of bronchitis? could maybe sugguest something about his lung tissue in general (from birth) etc. more succesptible to certain adult diseases or conditions or even more succeptible to certain environmental conditions that wouldnt have had a huge impact on another person.

another regarding the meds...going with what debbie said in the first post how long have you been on them and during that duration have you experienced any of the noted adverse side effects? no leading questions though i know :) maybe just ask him if he has been feeling the same or "funny" after taking the meds
